Medication Forms

Allergy and Anaphylaxis Action Plan and Medication Orders

If your student has a severe allergy and anaphylaxis condition, you must complete and return this form the the CAL instructor before the student may participate in any program activities: Allergy and Anaphylaxis Action Plan and Medication Orders.

All CAL instructors are professionally licensed Colorado teachers and are properly trained to address student needs in the event of an emergency.
